We have personalized care for chronic diseases for 12 months duration. We are based on team-based care and work on achieving specific targets using personalised approach. We advocate 4P’s in healthcare delivery. ( Predictive, Preventative, Personalised and Participatory).

We have completed two batch of patients and currently recruiting patients for third cohort. Please WhatssApp us and we will contact you. We only take 20 patients per batch.
